How the referral pathway works.

    If the form feels difficult, call 0417 465 872 and the team can help complete it with you over the phone.

    The team reviews preferences, goals, location and worker fit before confirming the next steps.

The team can use your preferences, goals and feedback to help shape the support match.

Worker preferences

Preferences are considered during intake, and worker bios or profiles may be shared before meet-and-greets or regular shifts.

Changing support

If a worker does not feel like the right fit, honest feedback is welcomed so the team can look for a better match.

Changing goals

Support needs can change. Monthly goal-setting reviews help keep care plans aligned with NDIS and personal goals.
